Custom install...possible?

Alright so I'm relatively new at this, but here it goes. I'm developing a program in VB.NET, and right now I publish the app and install it on my computer and other computers to test it. I'm alright with the installer that VB.NET Express gives you, but I'd like it to be something different. Is this possible?

If someone could please point me towards a tutorial or something that will tell me what I'd like to know, that would be greatly appreciated.

I was just reading another post that said the "ClickOnce" installer that comes with Express does not allow for custom installs. It was a thread from about a year ago though, so i don't know if anything has changed.
