Generating Database Diagram Docs

I've seen an output from a program called RapTier. Curious if anyone uses this, or anything else to get a documentation/view of a database schema such as SQL Server DB's?

Is "RapTier" the one to get or do you suggest another?

You could use Visio, erwin, or tools like dbartisen (I think thats what it's called)
