Standard Request for Proposal?

I'm in the process of starting a small software company targeting upstart businesses as well as promotional software for larger companies; as of yet our company has no standard Request for Proposal and I'm not sure where to start.

Any links to a template or even just some common questions for customers regarding a given project would be much appreciated.
