Index out of range, and another error

I had this type of thing working before, but I had a computer crash and lost the project. I'm using a string collection to populate a listbox and a combo box. As soon as the form loads, it's giving me an "index out of range" error. If I hit continue, it's doing that thing where it's also putting a blank line in the boxes. It doesn't save my second zip code if I add two either. I'm not really sure where the problem is. I think it's in one or both of these two events.
