Code for getting user name and password

hi to all.. im new vb.net.. i need a code for getting user name and password... only 3 times has allowed to get the user name and password.. above 3 times.. the message box has display invalid user name the form has closed... if they given the right user name and password.. new form has open.. in that getting name, address, and ect..after finishing this form click ok.. then finalyy display the name . address and etc in the message box... plz elp me
