add new record to ms access database

I want to create a register form.The user must key in his name in a textbox and click on register to add a his record in the access database.I've attached my access database for reference.I want to create the form like in my attached file.Please help me,i've tried a lot of websites but can't find the solution.

I've followed the steps in the tutorial but when i click save, the changes is not saved to the database.How come?
